Voluntary Self-Exclusion Agreement
By entering into a self-exclusion agreement, you confirm that you are over 18 years old and meet the legal eligibility requirements for this programme. Upon receiving your details, we will confirm the start of your self-exclusion. This agreement will prevent any deliveries to your address in your name, regardless of who places the order.
Please note that by providing this information, you are entering into a legally binding agreement with Booze Express. Both parties are required to adhere to the terms of the self-exclusion agreement. The Liquor Act mandates that reasonable efforts will be made by the licensee to enforce this agreement. However, it is your responsibility to ensure compliance with the terms you have agreed upon.
